If a job is open, you need to fill out a City of Los Angeles Application Form(Click Here for a copy of the form in PDF format), along with any supplemental forms required, and return it to the Personnel Department, or a designated satellite office, on or before the closing date listed. The Notification Card captures the following information: the job title you are interested in and your home address. These cards are kept on file for one year. If the job you are interested in should open within a year of your turning in the Notification Card, we will send you a notice - usually the job announcement and a City employment application. 3. Once you have turned in your application, you will be notified by mail of the date, time and location of the examination of the job for which you applied.
